if you indeed meant Jon's collection (or DJ's or mine, reached through the banner), that is phpdvdprofiler. You can get it here; support is also provided in that forum. Setup help is provided here. You will need a website with PHP and MySQL for it; alternatively you could run it locally only using a local server.
